Hugh Chatham Memorial Hospital Announces National Award for its Affiliate, Yadkin Valley Home Health
Elkin, NC, August 13, 2018 – Hugh Chatham Memorial Hospital is pleased to announce that Yadkin Valley Home Health has been awarded the 2017 Premier Performer for Home Health Patient Satisfaction, by the nationally acclaimed SHP (Strategic Healthcare Programs) Group. The award recognizes Yadkin Valley Home Health for achieving an overall score that ranked in the top 5% out of 4,500 agencies in the SHP national HHCAHPS benchmark as measured from the patient’s point of view.
SHP is a leader in data analytics and benchmarking that drive daily clinical and operational decisions. The annual SHP Best™ program was created to acknowledge home health agencies that consistently provide high quality service to their patients.
