In partnership with physicians, we design a care plan for the individual’s short or long-term home care needs. We then coordinate implementation and regularly review the plan to assure that needs continue to be met.
Our nurses evaluate patients by collecting clinical data, such as blood pressure, heart rate, oxygen saturation, temperature and weight, providing comprehensive programs for diagnoses including COPD, CHF and pneumonia. Our staff can also assist in chronic pain management, daily weight checks and a falls program, creating the optimal atmosphere for achieving patient-driven goals. If there is a question or concern, a home health specialist is available 7 days a week to assist with the provided care plan.

SERVICES WE OFFER:
Our comprehensive home health cardiac program is carefully designed to enhance independence and long-term health. We work with patients to control symptoms, improve self-care and at-home safety, and minimize ER visits and hospitalizations. In addition, the program includes therapy treatment with emphasis on improved overall independent function.
We work closely with each physician to create an individualized treatment plan that meets the patient’s goals. Our team will provide scales, initiate daily calls for follow-up weight checks and provide patients with a medical social worker (when ordered by MD) to assist with community resources.
Our clinicians are trained in memory loss diagnoses such as dementia and Alzheimer’s disease, and are prepared to provide emotional, clinical and community support for patients and caregivers in the home.
Our nurses, physical and occupational therapists communicate directly with orthopedic surgeons to ensure that all protocols are executed and individualized for each patient. This allows for an optimal experience and surgical recovery time.
Our therapy driven pulmonary program is designed to reduce symptoms of pulmonary disease, improve breathing ability and reduce shortness of breath. Our clinicians strive to increase overall strength and exercise tolerance and encourage participation in daily physical and social activities.
Our nurses are trained in assessing, evaluating and identifying patients who are at risk or need treatment for pressure ulcers, surgical wounds, lower leg issues (diabetic, venous and arterial) and burns. They assist in wound care management and provide treatment plans for each affected patient.

Our physical therapists are trained to help you regain your independence. Clinicians train patients and/or caregivers to relieve pain, develop or restore function, and maintain maximum performance, utilizing teaching, exercises and applicable therapy treatments.
Our occupational therapists gather background data, select and modify evaluation tools, and evaluate patients determining the level of patient’s physical and social functioning. Clinicians observe each patient’s activities of daily life and provide them with exercises and and in-home safety plan that guides them during recovery.
Our speech therapists are licensed speech-language pathologists who provide assessments and reassessments of patients and use those evaluations to provide therapy goals specific to each patient and their diagnoses. Our clinicians are experts in stroke recovery and work with hospital stroke coordinators upon discharge, allowing a seamless transition of care, ideal for patient and caregivers.
Our social workers help navigate challenging healthcare and socioeconomic factors, allowing patients to recover in a positive and supportive setting.
