Monday – Friday 8:00am-5:00pm

The QA LPN will be responsible for reviewing clinician documentation, care-plan, OASIS, etc., to ensure compliance according to agency, state and federal regulations. Ensure services are delivered according to physician orders and the plan of care. Provide QAPI education or documentation training as necessary to field staff.

Provides home nursing care, under the direction and supervision of an RN, based on agency policies and procedures, through the competent application of the nursing process in the home.

Monday – Friday 8:00am-5:00pm. ($10,000 Sign On Bonus), HCMH Outpatient Therapy Services.

The Occupational Therapist is responsible for completing evaluations, developing treatment plans, and providing ongoing treatments that are designed to improve the patient’s functional status in conjunction with the overall treatment objectives. Experience with splinting preferred.

Provides administrative, clerical and primarily, billing support to the agency.
Under the supervision of the Office Operations Manager, and Executive Director, assists in planning, organizing, implementing, and evaluating the activities occurring in the office by performing clerical and receptionist duties.
Reimbursement Coordinator will perform a variety of duties relating to patient communication. This will include reporting observations to the case manager in charge of the patient.

Education and Formal Training:  High School graduate or equivalent, Associate Degree preferred

Work Experience: Healthcare billing experience required, home health clerical/billing experience preferred.
